How to complete a form
Please follow our step by step guide to completing a form.
Step 1
The
forms made available on this site are Adobe PDF's. You will need Adobe Reader
to
view these forms. Clicking the image will take you to Adobe's website,
where you can download Adobe Reader for free.
Step 2
Click on the form title you wish to complete.

Alternatively, you can save a blankcopy of the form to your computer by right clicking the link with your mouse and selecting SAVE TARGET AS ....
Please note, that only blank forms may be saved. Completed forms cannot be saved to your computer.

Step 3
Place your mouse over the form field and left click. A blinking cursor will now appear in form field.

Step 4
The following warning message may appear when you place your cursor into the form field. Please click CLOSE.
Blank forms can be saved from the website to your computer (this saves you having to access the website everytime you require a blank form). However, completed forms cannot be saved to your computer.

Step 5
Type your responses directly into each relevant form field. To move from one form field to another either press the TAB key or use your mouse.

Step 6
Warning: keep all the text within the set height of the form field. If you enter too many lines of content into a form field a scroll bar will appear. Any text below the scroll bar will not appear when the form is printed.
If a scroll bar does appear, you must remove text until the scroll bar disappears.

Step 7
After completing the form please check to ensure that all information is accurate.
- Click the FILE menu on the top left hand side of Adobe Reader.
- Select the PRINT

Step 8
A Print window will appear, please select the printer you wish to print to and press OK.

Step 9
Please sign and date the completed printed form and send to the Charity Commissioners.
